If you have not already done so, reach out to your primary veterinarian, along with any specialists or surgeons who have treated your pet. Please request that they forward your pet’s complete medical records, including doctor's notes, lab work, specialized testing, and radiographs, to info@sunrisepawsrehab.com

*Ensure all vaccination records are included. All patients must be up to date on Rabies vaccination unless a valid veterinary exemption is provided.

Once all patient information is received, we will proceed with scheduling your pet's first in-home appointment!​
