If you have not already done so, reach out to your primary veterinarian, along with any specialists or surgeons who have treated your pet. Please request that they forward your pet’s complete medical records, including doctor's notes, lab work, specialized testing, and radiographs, to info@sunrisepawsrehab.com​

Once all patient information is received, we will proceed with scheduling your pet's first in-home appointment!​
